How to prepare a quality report on a noticed violation?

  • If you want to report openly, provide complete and accurate contact information. The law enforcement authority will contact you in case of additional questions.
  • Present the available information about the violation in as much detail as possible, specifying information about the time, place, involved persons, as well as information about possible witnesses or objects (for example, documents) that confirm the incident!
  • In the submission, indicate:
    • Who is the infringer;
    • What proves that a violation has occurred;
    • Which provisions of the law have been violated (if it is difficult to determine which of the provisions of the law have been violated, the institution, after evaluating the situation described in the application, will identify them itself, but this requires a sufficiently detailed description of the situation, which allows to understand the nature of the possible violation);
    • Facts that show that you are an interested person in the prevention of the infringement (whether and in what way you have suffered from the actions of the infringer);
    • What actions have been taken so far (or what you did to end the violation before contacting the institution. The institution needs this information to better understand the situation);
    • date of submission;
    • information about the submitter (if not reported anonymously).
  • If the submission contains information of restricted access, it is necessary to indicate precisely the information, data, figures, attachments, calculations, etc., which must be kept confidential. In practice, it is easiest to do this by highlighting (underlining or coloring) the specific data and indicating at the end of the submission that the highlighted information is of restricted access.
